Individual, Group, and Family Cognitive Behavioral Therapy and Its Limitations

Types of CBT

Cognitive-behavioral therapy (CBT) is one of the most commonly used approaches in psychiatry, and it is critical to understand the differences among its individual, group, and family modalities. The first strategy is the most personalized method because it allows for customizing psychotherapy techniques to a person’s needs and objectives (Huibers et al., 2021). It is also associated with the largest degree of control over one’s thoughts and behaviors.

On the other hand, group CBT is less personalized and controlled due to interactions among patients (Guo et al., 2021). It still has multiple benefits, such as social support, cost-effectiveness, and positive peer modeling, which are crucial advantages for most patients (Guo et al., 2021). Finally, family CBT can be delivered in both group and individual settings, depending on the specific objectives of patients within a family structural unit (Peris et al., 2021). Ultimately, it is critical to understand the application of various CBT types for maximum psychotherapy effectiveness.

Limitations of CBT

Consequently, it is critical to note that all CBT modalities have limitations and drawbacks as well. For example, group CBT is sometimes criticized for its complexity and lack of focus (PsychExamReview, 2019). These two challenges arise from the social nature of group approaches, which entail fluctuating relationship dynamics (PsychExamReview, 2019).

Furthermore, it might be exceedingly difficult for PMHNPs to effectively curate the sessions when patients are unwilling to cooperate, have specific mood disorders, or are unable to open up about their problems. For this reason, Guo et al. (2021) note that individual CBT is generally more effective for treating anxiety/mood conditions in adolescents compared to group CBT.

On the other hand, some experts believe that specific situations, such as communication among adult patients suffering from eating disorders, are more appropriate for group CBT (PsychExamReview, 2019). The ability to relate to others with similar problems is a relevant source of motivation that helps mitigate various issues. Ultimately, while group CBT can be challenging for many PMHNPs, it remains an extremely effective methodology in specific cases.
